Disaster Relief Clinic for Long Term Needs

When: Thursday April 30th
Time: From 9:00 a.m. to 8:00 p.m.
Where: At the Polk County Fair Grounds

Please come and fill out an assessment form and let us help you with your long term needs such as food, clothing, furniture and other household items. These needs are not covered by FEMA. Forms will be turned in to local churches and organizations for needs assessment and distribution of items.
